Who We Are & What Is Chronic Care Management?

Assurance Medication Management (AssuranceMM) works with your doctor’s office to provide Chronic Care Management (CCM) — additional support for patients living with ongoing health conditions.

CCM is a Medicare-covered service designed to help eligible patients who are living with two or more chronic health conditions receive ongoing support between regular doctor’s visits.

Managing multiple health conditions can mean keeping track of medications, seeing different healthcare providers, monitoring symptoms, and trying to stay on top of your treatment plan. That’s where we come in.

Our clinical team works alongside your doctor’s office — not instead of your doctor — to help coordinate your care and provide support throughout the month. Your doctor remains responsible for your medical care and treatment decisions. We provide an additional connection between you and your healthcare team when you need support between office visits.

What Can I Expect?

If you participate in CCM, a member of our clinical team will check in with you regularly to see how you are doing and whether you need additional support. Depending on your individual needs, we may help you:

• Check in regularly to see how you’re doing
• Review your medications and answer medication-related questions
• Help you understand how and when to take your medications
• Identify potential medication concerns, such as possible side effects or medication interactions
• Keep your care plan up to date based on your needs and goals
• Help coordinate information between your primary care provider and specialists
• Monitor changes in your health and help identify concerns that may need attention
• Help you understand when you should contact your doctor
• Connect you with appropriate community or healthcare resources when needed
• Help you stay on track with your treatment plan and health goals

When something needs your doctor’s attention, we can communicate with your healthcare team so they are aware of important concerns.

No Extra Office Visits

One of the conveniences of CCM is that you generally do not need to come into the doctor’s office for your CCM services. Most CCM support is provided conveniently by phone.

You continue to see your doctor for your regular appointments and medical care. We provide additional support between those visits.

Medication Support

Managing several medications can be challenging, especially when you have prescriptions from different providers or take both prescription and over-the-counter medications.

Our clinical team can help you better understand your medications and identify questions or concerns that may need to be discussed with your healthcare provider. For patients managing chronic pain or taking multiple medications, this additional support can be especially helpful.

We do not prescribe or change your medications. Any changes to your treatment or medications are made by your healthcare provider.

Your Care Plan Is Personal to You

Everyone’s health is different. As part of CCM, your healthcare team develops a personalized care plan based on your health conditions, medications, needs, and goals.

Your care plan helps keep everyone involved in your care working toward the same goals and provides a roadmap for managing your health over time.

What Does CCM Cost?

Chronic Care Management is a Medicare-covered service for eligible patients. Medicare generally covers a portion of the cost. Many patients who have a Medicare Supplement or other secondary insurance may have little or no out-of-pocket cost, depending on their individual insurance coverage.

Before you enroll, you will be informed about any cost-sharing that may apply to you. If you have questions about your specific coverage or potential costs, our team can help you understand what to expect.

You Are Always in Control

Participation in CCM is voluntary. If you decide that CCM is not right for you, you can stop participating at any time.

Your information is used and shared appropriately to support your healthcare and in accordance with applicable privacy requirements.
