AssuranceMM clients on average see that one staff member can manage 400 CCM patients per month. Compare this to the national average of 47 patients if your staff does CCM over the phone. AssuranceMM clients do not hire new staff members when kicking off. Rather, existing staff can utilize AssuranceMM to document the work they are already doing while also performing CCM efficiently in order to drive extra reimbursement and improve quality of care.
Typical Medicare cost-sharing is applicable to CCM, however, the large majority of Medicare patients also have a supplemental plan which covers co-pays. Our clients see that even the patients that do pay a co-pay continue with the program because they value the proactive care management and support.
AssuranceMM makes it easy to get the comprehensive, automatic CCM documentation from Assurance into your EMR so you don’t have to do any double documentation. We have a variety of solutions to help clients do this, including solutions for popular EMRs such as Athenahealth, Allscripts and NextGen.
FOR PATIENTS
CCM is a program built for individuals that participate in Medicare living with multiple long-term, or chronic, health conditions, such as asthma, arthritis, high blood pressure, heart disease or diabetes. If you qualify for the program, you can receive 20 minutes of remote care management on a monthly basis from a qualified healthcare provider. CCM services include:
- Personalized attention by your Chronic Care Coordinator each month.
- A direct line of communication between you and your AssuranceMM
Pharmacist. - Consistent help and encouragement with any and all health goals you
may have.
Based on your health profile, your healthcare provider has identified you as eligible for care management services. These services are provided on a monthly basis via phone and or mail and are in addition to your regular office visits. Your privacy in all these exchanges is secure and in compliance with the Health
Insurance Portability and Accountability Act (HIPAA).
The CCM program provided by AssuranceMM has been shown to have a positive impact on health outcomes for its participants — including fewer emergency visits and hospitalizations. The goal of the program is to anticipate and address health concerns or complications earlier when they are easier to
treat, or even prevent.
